The Next Generation of Dental X-Rays

Fifteen years ago I introduced digital imaging into my Glendale dental practice to improve diagnostic capabilities and reduce the radiation requirements of each exposure. Now we are introducing the latest in radiographic technology called 3D Cone Beam Technology. This technology allows me to capture and reproduce the detailed 3D anatomy of the skull in a reliable, affordable way, while emitting low doses of radiation. Using the combined advancements in CT scanning and 3D Sterio-lithic surgical and prosthetic computerized reconstruction, I am continuing my commitment to my patients of providing them with the most advanced and safest technology available.

Cone beam imaging is one of the most important breakthroughs in dental radiology, and has proven effective for a wide range of care and treatment applications. Our dentist located in Glendale states that cone beam technology has several features that make high-resolution, 3D imaging a reality. A digital x-ray scanner is mounted on a rotating arm that circles the patient’s head. As it rotates, the x-ray is projected in a carefully controlled, cone-shaped beam through the patient and onto an amorphous silicon flat panel or image intensifier sensor. The beam encompasses the patient’s entire head, so it only takes one pass to capture the complete skull anatomy. The resulting images are displayed on a computer screen.
The Benefits

Dramatically lower radiation emission: An average CT scan is about 600-700 microsieverts, while cone beam imaging is usually less than 70 microsieverts.
Short scan and reconstruction times: Scans take an average of about 20 seconds, and less than a minute later, images are reconstructed on the computer screen allowing to see—in 360°—undistorted, virtual, rotating models of the patient’s anatomy.
Better diagnosis: Bone thickness can be accurately measured to better determine implant candidates and better diagnosis of diseases or conditions.
The Cone Beam images have more intense details: Soft tissue, missing teeth, location of the nerve canals and the relationship between proposed implants and the opposite jaw are fully visible.

Placement of Dental Implants

Dental implants are traditionally completed in two phases, although technology is improving the method and making it possible to have your implants sooner and with less discomfort and inconvenience at the dentist office. This time we will describe how this treatment is commonly carried out:

Phase 1

This process is generally performed in the office with local anesthesia or light sedation to allow complete comfort to the patient; this is known as the actual phase of implant. The dentist will use precise and gentle surgical techniques, in order to place the implants into the jawbone during 3 to 6 months, allowing the Osseo integration to take place. This step will ensure a solid foundation for replacement teeth. During this time, temporary bridges or dentures may be used to minimize any appearance or chewing inconvenience.

Phase 2

This phase involves creating and placing the new tooth or teeth to the anchored implant(s) in your jaw. You can now enjoy your dental implants and feel as they are your natural teeth.

Making a good first impression with good Dental Health

There is no question that the health of your gum tissue has serious systemic consequences. I can tell a lot about a patients overall general health just by a 30 second scan of the gingival or gum tissue.

Plaque forms rapidly in the oral cavity. Within hours, bacteria colonizes on the gum tissue and teeth by secreting a mucopolysaccharide that if left undisturbed, will mineralize to form calculus or tarter. These bacteria use the food that we eat for their metabolism. The end product of their metabolism is an endotoxin that once secreted in the area attracts cells in our body that are the major clean up police of our immune system. This is called an autoimmune response to a foreign molecule. Because the cells are unable to reach the endotoxins, they inadvertently begin to destroy the healthy cells of the gum tissue.

This process or "disease" is called gingivitis. The gums swell and become bright red and bleed easily as a result of tenacious and efficient immune system. Being an action of an to our own cells, we label this process an autoimmune disease. Research has shown that this process greatly effects the liver, kidney and cardiovascular system. In fact, the American Heart Association sent out a bulletin stating that gingivitis and periodontal disease and the events that occur as a result of ignoring the problem, increases the patients chance of heart disease.

Despite the obvious risks of increased incidence of a heart attach and long term damage to the entire cardiovascular system, there are social implications as well. One of the side effects of this disease is bad breath or halitosis. First impressions are critical when meeting a potential bow. There are several subliminal signals that occurs in the first few minutes of the encounter that can either make or break a potential relationship or job interview. The oder produced by gingivitis is repulsive and sends a message to others that
"I am an unclean person with poor personal hygiene". This disease also has visual side effects. The red and inflamed gums make the person seem older and unkept. These visuals signals are something that everyone responds to without even knowing why they are turned off by the person effected with this problem.

The good news is that this is an easy problem to eliminate.

Regular visits to the dentist followed by proper oral hygiene virtually eliminates the disease in most cases. Left unattended, gingivitis will lead very quickly to periodontal disease. Periodontal Disease is an advanced form of gingivitis that requires surgical treatments as well as more frequent visits to the dentist that end up costing the patients thousands of dollars and in some cases the loss of all their teeth.

It makes since that one would brush and floss twice a day properly. I say properly because most people are not brushing the oral cavity in a way that will inprove their oral health.
Believe it or not, the most important structures to focus on while brushing your teeth are the tongue and gums. The teeth are almost self-cleansing! Focusing on the junction between the gum and teeth attacks the area that 95% of the plaque accumulate. The tongue is a great hiding place for the bacteria as well. In addition most people don't replace their tooth brush as often as they should. Ask your dentist for several tooth brushes at your next cleaning appointment and replace them every month.

Consider the amount of bacteria that is just having a carnival on a tooth brush that has been in use for six months or longer!

Proper brushing should be preceded by proper flossing. That is the only way that we are able to clean and smooth the inter-proximal areas of our teeth. Probably the best time to floss is before going to bed. If you forget to floss in the morning make sure to pay particular attention to the flossing time when you brush your teeth before going to bed. The teeth at night have very little or no mechanical stimulation during the hours that we are asleep. This can cause the plaque that has organized in the daily hours to begin to mineralize and form tarter or Calculus. Calculus can only be removed professionally by the hygienist.

There is no question that paying attention to these simple and cost effective personal hygiene steps are essential to ones general health and personal presentation. The next time you meet someone for the first time, be aware of these subliminal signals and see if they are obvious and powerful. I think you will see that it is a much larger aspect of your impression of the other person than you realize.

Payment for services is due at the time services are rendered. We accept cash, checks, and for your convenience, MasterCard, Visa and Discover. We will be happy to help you process your insurance claim for your reimbursement as long as you provide us with all necessary information in doing so.

In special instances, we may accept assignment of insurance benefits. However, you must understand that:

Your insurance policy is a contract between you, your employer and the insurance company. We are not a party to that contract and cannot guarantee any payment by your insurance company. Our relationship is with you, not your insurance company.
All charges are your responsibility whether your insurance company pays or not. Not all services are a covered benefit in all contracts. Some insurance companies arbitrarily select certain services they will not cover.
Fees for these services, along with unpaid deductibles and co-payments are due at the time of treatment.

Do you Drink mostly Bottled Water?

Are you one of those people that can’t leave the house without your bottle of water? And is that the only kind of water you drink? Then, you need to know that most bottled waters lack fluoride which is commonly found in public water and as we all know; the myth that "only children need fluoride" has been busted.

Sometimes we need more fluoride than what water and toothpaste can provide us and that's when we need to consider having a fluoride treatment by our Pasadena dentist.

How do I know if I may need a fluoride treatment:

A fluoride treatment consists of applying a varnish, foam or gel into a mouth guard which you only have to wear for a few minutes. After this you can go home and keep on with the treatment using recommended toothpaste or a prescribed gel which will ensure you receive the proper amount of fluoride you need. Dietary vitamins may also be advised.

If you are used to drinking mostly bottled water look at the label to make sure it contains fluoride! Also, the tap water your drink should contain the right levels of fluoride which should be between 0.70 mg/L and 1.20 mg/L. At Home Tooth Bleaching

Teeth Whitening

Hydrogen peroxide has been used for many years to bleach teeth. In past years, it was usually applied only by dentists, who used heat and/or light to accelerate its bleaching action. In recent years, a highly effective technique has been found that allows patients to bleach their teeth at home - although only when supervised by a dentist.*

The “at home” technique of bleaching teeth does not require heat or light, and allows you to monitor your own progress. The amount of color change depends on the length of time you wear the tray each day and the number of weeks you continue the treatment. The results can be unpredictable. Although effects may be noticed as early as a few days, many patients report that a minimum of two weeks is required to see a color change. Peroxide bleaching in custom-filled trays is thus the fastest and most painless method known.

Teeth Whitening Procedure:

One or two hours before bedtime, brush and floss your teeth, in order to clean them before placement of the tray. Rinse well.
Place one or two drops of bleaching gel in the tray at each tooth position to be bleached. Use the gel only in the tray provided by us to fit your teeth.
Place the tray firmly over your teeth. Spit out excess gel and foam, and wear loaded tray for one (1) hour. Then remove the tray, wipe it with tissue paper to remove the gel (do not wash it out at this time), reload the tray, replace it, and wear it for another hour.
Anytime you remove the tray for longer than 15 minutes, place new bleaching gel in it before you use it again.
Re-apply the bleaching gel approximately every 3-4 hours.
To receive maximum bleaching potential, the tray should be worn a minimum of twelve (12) hours per day.

Possible Teeth Whitening Side Effects:

Some patients have reported that teeth become sensitive to cold and biting pressure for a short time during and after treatments. The discomfort normally does not last for an extended time. If, however, your teeth continue to be sensitive, stop using the bleaching gel and call us.
Your gingiva or gums may react to the bleaching gel by creating gum sensitivity. If it does, simply decrease the amount of bleaching gel in the tray, or return to the dental office in Glendale so we can trim the appliance in order to reduce the amount of bleaching gel in contact with your gum tissue.
If bleaching trays are worn too long during any given day, discomfort in your jaw points may occur. This discomfort will go away when the bleaching trays are removed.

* In the past, hydrogen peroxide was considered to be a cosmetic agent only. However, the US Food and Drug Administration is now considering registering this chemical as a drug to assure its proper use and dentist supervision.

The Benefits

Dramatically lower radiation emission: An average CT scan is about 600-700 microsieverts, while cone beam imaging is usually less than 70 microsieverts.
Short scan and reconstruction times: Scans take an average of about 20 seconds, and less than a minute later, images are reconstructed on the computer screen allowing to see—in 360°—undistorted, virtual, rotating models of the patient’s anatomy.
Better diagnosis: Bone thickness can be accurately measured to better determine implant candidates and better diagnosis of diseases or conditions.
The Cone Beam images have more intense details: Soft tissue, missing teeth, location of the nerve canals and the relationship between proposed implants and the opposite jaw are fully visible.

Cone beam imaging is undoubtedly essential as I strive for excellence in patient care. Its 3D detail and precision reveals what 2D can’t: the true spatial relationships, shapes and measurements of every structure in the human skull.

Comparative Chart

	Traditional film x-ray: Provides two-dimensional images of teeth and jaws when the patient is positioned properly in the chair. Images are often distorted and magnified, making it difficult to Distinguish one tissue or bone area from another more difficult. Additionally it exposes patients to radiation.
	Medical CT scanning: Reduced radiation exposure, but still a high radiation dose. Multiple revolutions. Provides panoramic images; with the ability for computer-;enhanced image quality.
	Cone beam imaging: Low radiation dose. One revolution with short scan and reproduction times. Provides panoramic images with intense detail not readily visible in 2D scans.
